There are three Wormadam forms: Plant Cloak, Sandy Cloak, and Trash Cloak.
Wormadam’s Plant Cloak form is a Bug and Grass-type Pokemon with a green-tipped beak, a pair of large leaves resembling arm-like structures near its head, and a leafy coat with white flowers. According to its Pokedex entry in the game, this Wormadam’s cloak, made from plants, is now merged with its body, similar to fur or skin. This merging is suspected to be an effect of the energy involved in evolution.
Wormadam’s Sandy Cloak form is a Bug and Ground-type Pokemon with a brown-tipped beak, a pair of chained brown rocks resembling arm-like structures near its head, and a tan-colored, sandy cloak dotted with reddish-brown rocks. Wormadam prefers Springy Mushrooms, Dazzling Honey, Hearty Grains, and Plump Beans, causing them to be useful distractions before catching, battling, or simply feeding it.
It will drop a King’s Leaf and a Lum Berry after the player defeats or catches it in the wild.

A Pokemon’s Pokedex entry is finished when it reaches Research Level 10. Once one of a Research Task’s objectives is attained, a Pokedex entry’s Research Level rises by one level. After completing a Research Task objective with a multiplier of x2, the Pokedex entry grows two Research Levels.
